The goal of this page is to share existing PSS custom forms for all clinics to use. Where possible, we try to maintain the most up-to-date version and share forms not available by Telus.
Each file downloads in a .CFM file type. The file folder will be zipped, it must be unzipped before PSS will allow you to upload it for use.
1. From the Records window, choose Settings then Edit Custom Forms
2. In the Forms window from the File menu, choose Import Form(s)
3. Navigate to where you have the .CFM file type and upload it.
4. The file is now uploaded and listed in your Custom Forms under the same name of the .CFM file type.
